This information is completely confidential and will not be released without your written consent. This information is required to customize the treatment to your health needs and to make sure the treatment is safe and specific for you.
Cancellation Policy
There is a 24 hour cancellation policy in effect. I understand emergencies happen, however if 24 hrs is not given. If you miss an appointment without the required notice given, the full charge of the appointment will be charged. Please respect the time that is allotted for your treatment.
Lateness Policy
